HOW TO UPLOAD & SUBMIT YOUR INTAKE FORMS
Save time on your first visit!
Upload your forms virtually, ahead of time.
To do so, follow these instructions:
1. From the ‘INTAKE FORMS LIBRARY‘ below, find the intake forms that apply to you and your condition.
2. Click the ‘Click to Open in New Tab‘ button beneath each form that you need. The form will open up in a new tab on your internet browser.
3. Fill out the form(s) in your browser window.
4. In the top-right corner of your browser window, click the ‘Download’ icon. It will ask you if you want to download with or without the changes that you’ve made.
5. Select ‘Download With Changes‘.
6. Once the file has downloaded to your computer, you can upload the filled-out form to the ‘INTAKE FORM UPLOAD‘ form below:

No Insurance? No Problem!
If you either do not have insurance or do not wish to use your insurance for your physical therapy services, we offer a $100.00 per visit, self-pay fee option. Please contact us to learn more!

We gladly accept the insurance plans listed below. If your plan is not listed, please call us at 337-264-9856 as we are always adding new insurance plans.
As a courtesy to our patients, we will verify your health benefits prior to your arrival to determine if there will be a copay, deductible, or coinsurance for therapy services.
For those patients who do not have health insurance or those who have maxed out their benefits, we offer a private pay plan. Please contact Boulet Physical Therapy & Wellness Institute today for more information on our fees for service and payment details.
